To change your name, bring your documentation of proof to the Office of the Registrar in Room 2130 in the Student Union / RSSC and complete a Change of Address/Name form. You may also send a request in writing which includes your signature and a copy or copies of documentation of proof.

Proof of documentation:
Court documents (i.e., marriage license, divorce papers, adoption papers, etc.) OR

Driver's License AND Social Security card - both showing new name.

If sending request in writing, please mail to Office of the Registrar, PO Box 1570, State University, AR 72467.

Address Change

Students have two options to change their address:

1. Come to the Office of the Registrar in Room 2130 of the Student Union / RSSC and complete a "Change of Address" form.

2. Login to AccessPoint and select "Self Service Banner" in the left column.
Select "Student, Faculty, and Staff Login" and login.
Select "Personal Information."
Click "Update Addresses and Phone."

Student ID Number Change

To change your student ID number, a copy of your social security card must be mailed to the Office of the Registrar, PO Box 1570, State University, AR 72467. You may also come to the Office of the Registrar, which is located in Room 2130, Student Union/RSSC building.
